May is here! And what does that mean?  Skin Cancer Awareness Month! With 3.5 million cases diagnosed annually, skin cancer is the most common form of cancer in the United States.  That’s why this month it is important to spread the education of skin cancer and importance of sun safety.
<br/>

Did you know…?

<br/>
+ There are three main types of skin cancer:  basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) are the two most common, and melanoma which is the most severe.
<br/>
<br/>
+ One in five Americans will develop skin cancer in their lifetime.
<br/>
<br/>
+ Actinic keratosis is the most common precancerous lesion; 65% of all SCC arise from actinic keratosis.
<br/>
<br/>
+ Melanoma is the most common form of cancer for young adults 25-29 years old.
<br/>
<br/>
+ People who have more than 50 moles, atypical moles, or a family history of melanoma are at an increased risk of developing melanoma.
<br/>
<br/>
+ Exposure to tanning beds increases the risk of melanoma.
<br/>

Living in Florida, we get the advantage of a warm climate with hot sun rays.  However this leads to frequent sun exposure.  Chronic ultraviolet (UV) exposure leads to breakdown of DNA synthesis and sun damage, increasing the risk of skin cancer production.  Anyone can get skin cancer.  It is important to know your skin and check for any moles or new lesions that may “pop” up or have changed. Using the ABCDE guideline will help point out any suspicious moles that need medical attention. The ABCDE guideline stands for: Asymmetry, Border, Color, Diameter, and Evolving.
<br/>
<br/>
Asymmetry – if you cut the mole or lesion in half, and one half is different from the other
<br/>
<br/>
Border – poorly defined borders or irregularly shaped
<br/>
<br/>
Color – multiple shades of tan, brown, or black and sometimes white, red or blue
<br/>
<br/>
Diameter – usually larger than 6mm or a size of a pencil eraser
<br/>
<br/>
Evolving – if it looks different from the rest or is changing in size, shape, or color
<br/>
<br/>
If you have any moles or a new lesion that has any of these ABCDEs, make an appointment as soon as possible with your dermatologist.  The earlier a skin cancer is detected, the easier better the prognosis.  Most skin cancers are treated with a simple surgical excision or Mohs surgery to remove the cancerous lesion and get clear margins.  Depending on how larger and deep the skin cancer is, your physician may also recommend topical chemotherapy agents, electrodessication and curettage, or localized radiation.

Melasma is a common skin condition that increases the pigment in the skin of sun exposed areas.  The condition is much more common in women; although about 10% of people diagnosed with melasma are men.  Most often brown or grayish-brown patches are seen on the face, commonly the cheeks, forehead, and above the lip.  This condition does not cause any symptoms that patients will feel, but affects their cosmetic appearance.  Many women call it a “mask.”
<br/>
<br/>
There is not a true etiology or cause of melasma.  The melanocytes, or cells that make pigment, produce too much color.  People with darker skin are more likely to get this condition because they have more active melanocytes than those with light skin.
<br/>
<br/>
Sun exposure is the most common trigger.  Ultraviolet (UV) light stimulates melanocytes, making the pigment worse and darker.  A change in hormones also plays a role.  Women who have been on birth control pills for a number of years, or who are pregnant will often note a brownish hue developing on areas of their face.  Sometimes after the hormone pills are discontinued or once pregnancy is complete the pigmentation may lighten.  But often times, the dark coloring will remain, leaving these patients concerned with this new appearance.  There are also some cosmetic skin care products that can be too irritating to the skin, causing inflammation and making the melasma worse.  It’s important to discontinue any topical products that may be irritating.
<br/>
<br/>
Melasma is often diagnosed clinically by your physician.  To see how deeply the pigment penetrates in the skin, your physician may use a device called a Wood’s light.  This helps with formulating a proper treatment regimen.  Sometimes a skin biopsy may be necessary for a definitive diagnosis.  This is done in the office where a small piece of skin is removed and sent to the lab for analysis.

<br/>
Melasma can be a frustrating condition to treat because some people have it for years, or even a lifetime.  There are many treatments available to help lighten the pigmentation.  The most common first line treatment is using a hydroquinone based product.  This is a bleaching product that minimizes the melanocyte production.  This can be used by itself, but is often times used in combination with other products, such as tretinoin, corticosteroids, azelaic acid, or kojic acid.  Together these active ingredients will exfoliate and lighten the skin.  However, some of these products can be irritating, therefore if you are noticing worsening of your pigment when using these topical medications, it is important to discontinue them immediately and consult your physician.  If topical medicine is not providing enough improvement, your physician may recommend physical procedures.  Some include chemical peels, such as a Jessner peel and Vi peel, microdermabrasion, and skin rejuvenation lasers.
<br/>
<br/>
Melasma is very stubborn and can take a few months of treatment to see improvement.  Many times the pigmentation will fade on its own, however any sun exposure will cause it to return and darken again.  So it is very important to keep treating the skin and using sun protection daily.  Wearing sunscreen with broad-spectrum protection against UVA and UVB rays, SPF greater than 30, and zinc oxide or titanium oxide ingredients will minimize the risk of melasma and prevent it from getting worse.

			<content:encoded><![CDATA[<h2><strong><em>A Blistering Rash, Could Be Shingles. Stefanie Gold, PA-C With Natura Dermatology  &amp; Cosmetics Explains Herpes Zoster, Also Known As Shingles.</em></strong></h2>
A 58 year old male comes in to the clinic with a group of small blisters on the right hip. These blisters have been present for 2 days. He is complaining of a “stabbing” pain in the area and over the counter cortisone has helped slightly. When asked about previous illnesses, he did admit to have flu-like symptoms for about 1 week. What is his most likely diagnosis?  Herpes Zoster (Shingles).

<br/>
Herpes Zoster, also known as Shingles, is a painful, blistering skin rash due to the varicella-zoster virus, the virus that causes chickenpox. Anyone who has had chickenpox can get Shingles. The reason being is that after the chickenpox clears, the zoster virus that causes it stays dormant in your nerve cells. Once that virus reactivates (wakes up) it causes the blistering disorder. This usually happens many years later.
<br/>
<br/>
We are not exactly certain why the virus reactivates when it does but we do know that you are more likely to develop Shingles if you are or have suffered through recent emotional or physical stress, had chickenpox before the age of 1, or have a weakened immune system, either due to illness or medications. Although it can occur at any age, it most commonly affects adults over the age of 60. If an adult or child comes in contact with shingles and they have not yet had chickenpox or the vaccine, they can develop chickenpox at that time. Usually this virus will reactivate only once but multiple cases are possible. If you do get Shingles more than once, the first one is usually the most severe.
<br/>
<br/>
This rash usually presents itself as red patches on the skin, followed by small blisters. Some people may complain of a tingling or burning sensation in the area before the rash appears. The blisters then break, forming small ulcers that begin to dry and form crusts. The crusts fall off in about 2 to 3 weeks and scarring is rare. Pain may be significant and can lasts weeks to months in some cases. Flu-like symptoms may also be present, although rare.
<br/>
<br/>
The key thing to remember about herpes zoster is that it will only affect one side of the body. The reason being is that the virus lives in the nerves of what we call dermatomes. A dermatome is an area of the skin supplied by nerves from a single spinal root. Each spinal nerve root supplies an area on one half of the body, therefore not allowing the rash to cross the midline. So, if you have a blister-like rash that is on both sides of your body (ex: both arms, across your abdomen) chances are, it is not shingles. The rash may also appear on the face, eyes, mouth, and ears. This can affect major nerves in the area and care should be sought immediately.
<br/>
<br/>
Usually, herpes zoster is diagnosed based on patient history and physical examination. If there is doubt, your health care provider may do a scraping of a blister to see if your skin is affected by the virus. Once the diagnosis is made, you most likely will be given an antiviral to help reduce pain and complications and shorten the course of the disease. For best results, the medications should be started within 24-48 hours of symptoms and preferably before the rash appears. Acyclovir, famciclovir, and valacyclovir are most commonly used. For less severe cases, a topical antiviral may be prescribed. If there is significant inflammation, a topical antiinflamamtory may be given. Cool, wet compresses can be used to reduce pain and soothing baths and lotions may help to relieve itching and discomfort. You should be cautious and keep open, oozing lesions covered. Although Shingles is much less contagious then chickenpox, it is highly recommended to keep any open blisters covered and those affected should stay away from pregnant woman and children to prevent spread of the virus. Remember, you are contagious until the blisters crust over, regardless of being on medication.

<br/>
Patients with Shingles rarely need a hospital stay but, although rare, there are some cases with serious complications; post-herpetic neuralgia (pain in the area where the shingles occurred), eye problems (if the disease affects the face), and secondary bacterial infections.
<br/>
*There is now a herpes zoster vaccine available called Zostivax. It is different than the chickenpox vaccine. Older adults who receive the herpes zoster vaccine are less likely to have complications from shingles. It is recommended now that adults older than 60 should receive the zoster vaccine as part of routine medical care.

Melanoma, the most dangerous type of skin cancer, is caused by cells that produce pigment, called melanocytes. These cells are responsible for skin and hair color. Melanoma is not as common as other types of skin cancer; however, the rate of melanoma is steadily increasing, especially in young adults.  According to the World Health Organization, melanoma is responsible for 75% of skin cancer deaths, killing about 48,000 people worldwide annually. One in 55 people will be diagnosed with melanoma over their lifetime; one person dies of melanoma every hour.  In this week’s blog, we will discuss possible reasons why this increase in incidence among young adults may be occurring.

<br/>
Melanoma is currently the 5<sup>th</sup> most common cancer in men and the 7<sup>th</sup> most common in women. Behind only breast cancer, melanoma is the second most common invasive cancer in young adults. Based on studies from the Surveillance, Epidemiology, and End Results (SEER) database, the National Cancer Institute reports that “in males aged 15 to 39 years the annual incidence of melanoma increased from 4.7 cases per 100,000 persons in 1973 to 7.7 cases per 100,000 persons in 2004. In females, the annual incidence increased from 5.5 cases per 100,000 persons in 1973 to 13.9 cases per 100,000 persons in 2004.” This is solid evidence that in both young adult males and females, melanoma continues to be a major cause of morbidity and mortality in the United States.
Another interesting finding was that in this age group, the incidence of cutaneous melanoma has increased more than 6-fold during the past 40 years with females having a slightly higher lifetime risk. This is a changing trend as the lifetime risk of developing melanoma is usually higher in men then woman. This finding may be explained by some sex-specific behaviors that lead to different UV light exposure. Young woman are more likely to engage in activities that increase the risk of melanoma, such as exposure to artificial sunlamps. In a survey conducted by the American Academy of Dermatology, it was found that the frequency of severe sunburns and tanning bed use had increased and was higher among woman, despite the public awareness of the harms of UV light exposure.

<br/>
In addition to UV exposure in adulthood, tanning bed use and sunburns in childhood and adolescence may contribute to the increasing rise in incidence of melanoma in young adults. “A recent study reported that people who use indoor tanning beds frequently are 74% more likely to develop melanoma, and we know young woman are more likely to use them than young men,” said Dr. Jerry Brewer, lead researcher and dermatologist with the Mayo Clinic. Again, despite public health education campaigns designed to decrease such behaviors, children, adolescents, and adults continue to put themselves at risk.

<br/>
The World Health Organization classifies indoor tanning beds as a human carcinogen. Why? Nearly 28 million people (2.3 million teens) tan indoors in the United States annually. Nearly 70% of tanning salon customers are Caucasian girls and woman, primarily aged 16-29 years. According to the Skin Cancer Foundation, across all age groups, males and females who have ever used tanning beds have a 15% higher risk of developing melanoma and people who first use a tanning bed before age 35 increase their risk for melanoma by 75%.  The International Agency for Research on Cancer, an affiliate of the World Health Organization, includes UV tanning devices in its Group 1, a list of the most dangerous cancer-causing substances. Other (<strong><em><span style="text-decoration: underline;">members)</span></em></strong> in this group include plutonium, cigarettes, and solar UV radiation. The indoor tanning industry has an annual estimated revenue of $5billion.

<br/>
What is so dangerous about artificial sun exposure?  “The indoor tanning industry has often claimed that tanning beds are safe because the bulbs have more UVA radiation than UVB”, says American Cancer Society Deputy Chief Medical Officer Len Lichtenfeld, MD. New high-pressure sunlamps may receive as much as 12 times the annual UVA dose compared to the dose received from natural sun exposure.  This is significant because it is now known that UVA rays penetrate to the deep layers of the skin, causing a tan, along with DNA change in the melanocytes, causing melanoma.

<br/>
In addition to UV exposure, risk factors for melanoma include having many moles or moles that have abnormal shape or color, fair skin, blue eyes, freckling, light hair, a family history of melanoma, and having received a severe or blistering sunburn as a child or teen. Using an SPF of at least 30 with UVA/UVB protection is one of the best methods of prevention against melanoma and other kinds of skin cancer. It is important to apply the sunscreen 30 minutes before exposure to allow for absorption, reapply every 2 hours, and reapply after submergence in water. Wearing sunglasses, hats, and seeking shade also helps.
<br/>
<br/>
Recognition of changes in the skin is the best way to detect early melanoma. The most common locations include the upper back, torso, legs, head, and neck. In young adult females the torso is now the most common location for developing melanoma, most likely due to high-risk tanning behaviors. Doing self skin exams and following a simple <strong>ABCDE</strong> rule may aid you in noticing the warning signs of melanoma:
<br/>
<br/>
<strong>A</strong>symmetry: one half does not match the other
<br/>
<strong>B</strong>order: irregular border, edges are blurred
<br/>
<strong>C</strong>olor: different shades of tan, brown, red, purple or black; multi-colored moles; moles changing color
<br/>
<strong>D</strong>iameter: &gt; 6mm (size of home pencil eraser); some melanomas can be smaller
<br/>
<strong>E</strong>volving: A mole looks different from the rest or is changing in size, shape, or color.
<br/>

Living in Florida, we are lucky to have year round warm weather.  But with warm weather brings perspiration.  And if you are dealing with excessive sweating, a warm climate can make you very uncomfortable.  But there is good news! If you suffer from hyperhidrosis (excessive sweating), Natura Dermatology and Cosmetics has treatment options for you to get you through these next hot summer months.
<br/>
<br/>
Hyperhidrosis is a condition in which one sweats more than is required by the body for thermoregulation.  There are two main types of hyperhidrosis:  primary focal and secondary generalized.  Primary focal hyperhidrosis refers to excessive sweating that is NOT caused by another medical condition.  It <em>IS</em> the medical condition.  This type of sweating most often affects the hands, feet, axilla (under arms), head or face, and tends to be symmetrical.  It mainly begins in early childhood.  On the other hand, secondary generalized hyperhidrosis is caused by another medical condition or may be a side effect from a medication, hence the name secondary.  People with this type of sweating typically experience their symptoms generalized over large areas of the body, and many times experience sweating while they sleep.
<br/>
<br/>
It is important to be evaluated by a physician to assess and distinguish which type of hyperhidrosis you have.  If it is thought that you have secondary generalized hyperhidrosis, the first step is to target and treat the primary cause.  A basic workup may include blood work of thyroid function tests and blood glucose level.  Hyperhidrosis is usually detected clinically.  However, your physician may use an iodine starch test that allows for a more accurate visualization of the excessive sweat.
<br/>
<br/>
Severe hyperhidrosis may lead to maceration of the skin often resulting in infections.  Not only is it a physical disease, but hyperhidrosis also affects many patients emotionally.  Increased sweating can leave patients feeling embarrassed by the stains left on clothing and bad body odor.  Some individuals will alter what clothing they will wear, or get stressed when meeting new people in fear of shaking hands with their sweaty palms.  This stress makes their symptoms worse which then leads to social restrictions and limited activities.  All in all, it is important to address this condition for a better quality of life.
<br/>
<br/>
There are multiple treatments options for hyperhidrosis.  The first and most simple step is using a topical drying agent.  Drysol, a 20% aluminum chloride solution, is often the first-line prescription of choice.  It is applied nightly directly to the affected area and usually reduced in frequency once improvement is seen.  Over the counter antiperspirants, such as Dove Clinical Strength, also contain a higher percentage of aluminum chloride and is an easy option for patients to try initially and for maintenance.  Some physicians may prescribe an oral medication, typically an anticholinergic, if topical medications have failed.  However, these medications are limited by their side effects such as dry mouth, dry eyes, urinary retention, and constipation.
<br/>
<br/>
Increasing in popularity is the use of Botox injections for the treatment of hyperhidrosis.  As a natural, purified protein, Botox temporarily blocks the secretion of the chemical that is responsible for “turning on” the body’s sweat glands and thereby decreases or stops the production of sweat in that area.  This is a very simple procedure done in the office, where small injections are made in the affected area.  There is little to no pain, although the palms tend to be slightly more sensitive.  The advantage of <strong>using botox to treat excessive sweating</strong> is that the treatment can last from 4 – 12 months.  How great would that be to have decrease perspiration for the entire summer! These injections, however, do not cure hyperhidrosis.  Your symptoms will go away gradually usually within a week and then return gradually.  Therefore follow-up injections are required to maintain dryness.  Many people are concerned that if they stop sweating in one area, they will increase in other areas.  Don’t worry.  Your body will naturally eliminate the sweat evenly throughout the body to maintain the proper thermoregulation needed.  There is no need to feel embarrassed or deal with increased sweating anymore.  Call our office and schedule and appointment for evaluation to see which treatment is right for you!

Rosacea, a common disorder frequently mistaken for acne, is a condition that tends to become more apparent in the summer months. It usually arises later in life than does acne but can mimic the symptoms of it. Both conditions look alike, often can respond to the same treatments, and they may coexist.
<br/>
<br/>
Rosacea is a chronic condition with no known cure. It usually presents in patients between the ages of 30-50 years old. It occurs most commonly in fair-skinned people and woman are three times more likely to be affected by men. The exact cause is unknown, but certain environmental factors are known to contribute to the development and progression of the disorder. These contributing factors include sun exposure, excessive face washing, and irritating cosmetics. Factors that have believed to be contributors but have no scientific evidence to support the thought include excess alcohol ingestion, emotional stress, spicy foods, smoking, or caffeine. Sun exposure is the number one triggering factor for progression of the disease!
<br/>
<br/>
The diagnosis of rosacea can usually be made based off of clinical exam alone. A facial eruption consisting of red bumps, pustules, and vessels is most likely the complaint. A rosacea patient lacks blackheads, whiteheads, and cystic lesions that are commonly seen in acne patients. The patient will usually describe their symptoms as redness beginning on the cheeks and forehead and later spreading to the nose and chin. “Flushing” or “blushing” is a common complaint. As the disorder progresses, vessels, bumps, and pustules begin to arise on the red background. There is usually no pain, however stinging has been reported. Rosacea of the eyes, also known as ocular rosacea, is possible, although rare. This may present as a foreign body sensation in one or both eyes.
<br/>
<br/>
In order to prevent the development or halt the progression of rosacea, patients should be advised to avoid certain contributing environmental factors. As stated above, sun exposure is the #1 trigger! Application of sunscreen before sun exposure and protective clothing is key to keeping rosacea under control.  If possible, long-term control of rosacea should be attempted with topical therapy alone. However, many people with rosacea have very sensitive skin and caution should be taken when prescribing these medications. Medications commonly used for acne such as topical retinoids and benzoyl peroxides may cause irritation in the rosacea patient and may actually exacerbate the symptoms. Common prescription medications that your health care provider may write for include metronidazole, azelaic acid, or sulfur-based medications.
<br/>
<br/>
Systemic therapy for rosacea should be reserved for initial control and/or breakthrough flares only or if treatment with topical medications has failed. The same systemic medications used to treat acne can be used to treat the papules and pustules of rosacea as well. Tetracycline and tetracycline derivatives such as minocycline and doxycycline are the first line choice of management. At the correct doses, the mechanism of action is most-likely anti-inflammatory rather than antibiotic, as there has been no evidence of bacterial causes of rosacea.
<br/>
<br/>
Other treatment options include Pulse Dye Lasers (PDL) and Intense Pulsed Light (IPL). These treatments are used to destroy the dilated vessels underneath the skin. Patients are instructed to avoid irritating cosmetics, astringents, and exfoliating agents and should lean towards gentle cleansers and water-based moisturizers such as Cetaphil and Cereve and cosmetics in which sunscreen is already added.

There are many factors that play a role in varicose and spider veins.  The main cause is heredity.  Chances are, if your parents have them, you will get them also.  Pregnancy is felt to be the second most common predisposing factor, possibly due to the increased pressure from uterine enlargement.  However, a link between increased hormonal changes from pregnancy, birth control, puberty and menopause and the formation of these veins have also been noted.  If you have a job that requires prolonged standing or sitting, you may notice the appearance of spider or varicose veins over a period of time.  This lack of movement makes it more difficult for your veins to pump blood back to your heart, which will lead to the dilation of the blood vessels.  Other possible etiologic factors include obesity, chronic use of tight clothing, trauma, and a medical history of weak vein valves.

This condition tends to worsen over time, so the sooner you come in for treatment, the easier and more effective the results. When you come in for your consultation, a detailed medical history and examination of the areas will be done.  This is to rule out any underlying medical conditions that should be addressed prior to treating any superficial veins.  Thick tortuous varicose veins may be an indication of venous insufficiency and an ultrasound or further examination by a vascular surgeon may be required.  Most patients, however, present with the small clusters of telangiectasias and the reticular veins, which are slightly larger with a dusky bluish-purple color, but not yet varicose.

Sclerotherapy is the gold standard of therapy in the treatment of spider veins, and has been used in Europe for over 60 years and over 30 years in the United States.   In Sclerotherapy, a sclerosant is injected into superficial blood vessels, which irritates and damages the vessels.  This will stop the transport of blood in that vessel and over time the body will absorb the damaged vein.  Most people will notice an improvement approximately four weeks after initial treatment and multiple treatments may be necessary.  Follow up with compression stockings is essential in the effectiveness of the treatment and may prevent new veins from forming.  Laser treatments can also be used to treat leg veins, but typically best used for smaller vessels or veins that have not responded to Sclerotherapy.

<h3>What Is Ultherapy?</h3>
First and foremost, Ultherapy is a non-surgical approach to tightening sagging or drooping skin.  Think of it as an inviting alternative to face lift surgery.  Ultherapy is an ultrasound treatment for counteracting the effects of time and gravity on your skin.  An average Ultherapy treatment on the face and neck is about 30-60 minutes.  It uses the body&#8217;s own regenerative response to gently and gradually restore memory to the skin and underlying tissue support.

With one treatment, the regenerative process is initiated, but the effect will build gradually over the course of two to three months.  Further improvements can even appear up to six months following a procedure.  Consider doing more than one Ultherapy treatment for optimal results.
